Clinical Affiliations and Shadowing

AOT offers shadowing and clinical affiliation opportunities to students interested in pursuing occupational and physical therapy careers. Applicants are accepted and assigned on a first-come, first-served basis, depending upon the availability of therapists to supervise.

Required Clearances and Forms

The following clearances/documents are needed prior to completing shadowing and/or clinical affiliation opportunities with AOT. All clearances must be completed within 12 months of your start date with AOT and should be for Employment NOT Volunteer:

  • Child Abuse History Clearance
    • Can be obtained here
  • FBI Fingerprinting
    • Click here
    • Enter the following service code: 1KG756
  • Criminal Record Check
    • Can be submitted online
    • Do not choose the ‘Volunteers Only’ option
  • Mandated Reporter Training
    • A certificate showing completion of 3 training hours is required
    • The University of Pittsburgh Social Work Department has an online course for this training https://www.reportabusepa.pitt.edu
  • Act 24 document
    • Can be obtained here
  • AOT confidentiality form
  • Clear digital photo of applicant
  • Additional requirements for Clinical Affiliations: TB test and CPR
